Please bring a photo ID with you at the time of your appointment.
You will not be seen without a photo ID.
Please bring your insurance card with you if you will be using
your health insurance. If you do not have health insurance, or
choose not to use it at the time of your visit, you will be charged
according to a sliding fee scale. If you have health insurance,
you will be charged your regular co-pay.
What is a sliding fee scale?
Through private donations and some government funding, PPCC is
able to provide services on a sliding scale. Fees for all eligible

may decline to provide this income and elect to pay the full fee.
